Have you ever been found guilty by a court or other tribunal of having committed a violent act against another person, engaged in any misconduct involving a minor or been convicted of a crime except for a minor traffic violation?  YES   NO

If YES, please be prepared to discuss your record with the Nature Centre Coordinator. Your record will be evaluated against the established guidelines in the High Park Initiatives' Police Record Check policy and a decision regarding your placement will be made.

 

Consent to a Criminal Record Check

Due to the vulnerability of the population served by the High Park Nature Centre, all of our staff and volunteers are required to submit a police check through the Toronto Police Services. The objective of the record check is to confirm a candidate's personal suitability to perform appropriately in a position of trust with vulnerable persons.

 

The cost is $15.90 for volunteers. You are responsible for paying this fee and dropping your form off at the Toronto Police Services at 40 College Street. Results of the record check will be sent directly to the High Park Nature Centre Coordinator and will be kept confidential - seen only by the Coordinator and the High Park Initiatives Human Resources Director. Should a record exist, the candidate's record will be evaluated against established guidelines set by High Park Initiatives and a decision regarding placement will be made.
